  • PDE-430:Student/Intern Teacher Evaluation form (USED for the FINAL Evaluation ONLY) (Pennsylvania Statewide Evaluation Form for Student Professional Knowledge and Practice PDE-430)
  • PDE-430:Student/Intern Teacher MIDTERM Evaluation (Use for the Midterm only. )
    •  Student Teachers & Coop Teachers Input-due before October 15, 2008 - Discuss and submit to Supervisor.  
    • Supervisors: Due Thursday, October 23, 2008, in Field Placement Office
    • Final PDE-430 [due by December 11, 2008 in Field Placement Office (Student Teachers & Coop Teachers Input due December 1, 2008)] is the same as the Midterm PDE-430, except that is has a place in the "Justification for Evaluation" Block for "Goals for Improvement."
