Pennsylvania Statewide Evaluation Form for Student Professional Practice/Performance PDE 430

Guidelines for the use of PDE 430 
by Holy Family University 
Student Teachers and Supervisors, and Cooperating Teachers.

The Pennsylvania Statewide Evaluation Form for Student Professional Practice/Performance will be used as the primary assessment instrument for the student or intern teacher during the student teaching experience.  The Form identifies four categories: Planning and Preparation, Classroom Environment, Instructional Delivery, and Professionalism.  Each category lists several areas that the student teacher is required to demonstrate a level of proficiency.  Demonstration of this proficiency needs to be verified by an appropriate “Source of Evidence.”  The  “Sources of Evidence” include lesson plans, observations, interviews, and resources, to name a few.

The form will be used as follows:

1.      By the student teacher:  The student teacher needs to be knowledgeable of the areas being assessed and provide evidence to the cooperating teacher and University supervisor during the course of student teaching.  The items under “Student Teacher’s performance appropriately demonstrates” need to be backed up by appropriate evidence.

A PDE 430 will be completed by the student teacher during the 6th week of student teaching, reviewed with the Cooperating Teacher, and submitted to the University Supervisor.  During the Midterm evaluation, "Goals for Improvement" are to be identified and documented in the "Justification..." block.  

2.      By the Cooperating Teacher:  The Cooperating Teacher will become familiar with the contents of PDE 430 and assist the Student Teacher in experiencing all of the performance areas.  At the 6th week and the 14th week, the Cooperating Teacher will complete a PDE 430 independently and then discuss it with student teacher, insuring that the University Supervisor receives a copy. During the Midterm evaluation, "Goals for Improvement" are to be identified and documented in the "Justification..." block  

3.      By the University Supervisor:  The University Supervisor will complete two official PDE 430s with input from the student teacher and cooperating teacher.   

A.     The first PDE 430 is to be completed by the 7th week of student teaching (Midterm)and submitted to the Field Placement Office.  This PDE 430 is a requirement of the Pennsylvania Department of Education and will become part of the student teacher’s file.  The mid-term PDE 430 will also satisfy the University’s requirement for the School of Education’s comprehensive examination. A minimum score of 4 points, and no scores of 0 in any category, is required to satisfy the comprehensive examination requirements.  During the Midterm evaluation, "Goals for Improvement" are to be identified and documented in the "Justification..." block

B.     The second PDE will be completed as part of the final student teacher evaluation.  Input from the student teacher and cooperating teacher will be included in this evaluation.  This PDE 430 will be submitted to the Department of Education as part of the official recommendation by the University for teacher certification. A minimum score of 4 points, and no scores of 0 in any category, is required to satisfactorily complete the requirements of student teaching.
