- PDE-430:Student/Intern Teacher Evaluation form (USED for the FINAL Evaluation ONLY) (Pennsylvania Statewide Evaluation Form for Student Professional Knowledge and Practice PDE-430)
- PDE-430:Student/Intern Teacher MIDTERM Evaluation (Use for the Midterm only. )
  - Student Teachers & Coop Teachers--due February 20 - Discuss and submit to Supervisor.
  - Supervisors: Due March 15.
  - This form is the same as the Final PDE-430, except that is has a place in the "Justification for Evaluation" Block for "Goals for Improvement."

What Praxis tests do I need to take in PA? (Because of the changes in PA requirements, the best way to check this is to go to the PA dept of Ed web site http://www.pde.state.pa.us click on Teaching in PA , then click on Testing Requirements ,and then click on Which PRAXIS Test(s) Do I Need?)
